[Ger-Poland-Volhynia] Kazimierka Church

I visited a rural church in Kazimierka near Grodziec. I was told it was a Lutheran church or had been at one time. I have photos of the church and the sign on it. The original writing on the sign has been replaced with Polish writing.

Does anyone know of a Lutheran church established in Kazimierka? Would someone be willing to translate the writing?
