[Ger-Poland-Volhynia] Need help with Translation

I want to thank everyone who responded when I asked for help with translation. 
This group has members who are always eager and willing to assist.
I've learned two of the documents are Immunization records, written in German, 
and confirm that my father attended school in Germany.
Father's birth record/certificate is in Old Russian and is attached to a hand 
written German translation.
