[Ger-Poland-Volhynia] Polish to English translation help requested

Hello all,

I would very much appreciate some help with a WORD-FOR-WORD  translation of
a handwritten birth certificate from a 1830's.

I have tried using Google Translate, but I'm having great difficulty making
out the hand writing.
