You might try the following links, which are maps showing the Lutheran 
parishes (see locations with a +), and their date of formation.   Be 
aware that the last two links will require the djvu plugin for your 
browser (that plugin can be downloaded at 
(use the small "here" on this page to download, or you will get a bunch 
of "free" stuff if you use the "Download Now" link)
