[Ger-Poland-Volhynia] Translation

I have received the application for naturalization  for the family of my 
great grandmother's brother.
It is a fill in the blank slightly over one page application in German.
I can make out some of it, but not all.
I see the names of my second great grandparents and I'm hoping there 
might be a birth location there:)
I can't make out the writing after their names.
Is there someone who would be willing to  translate the pertinent 
information from this application.
