[Ger-Poland-Volhynia] Help on a Name and Surname...DB

Hello Everyone !
Im having trouble reading a name on a death certificate. Any help would be 
greatfull.
Says.....Tehla or Tekla   ???  Te_la
Surname-  Szymusiah or Szym_si__

This person was the mother of Victoria Ciszek.
It also states that she was born in Zolcina, Poland. I cant find this place, 
Zolcina anywhere. Anyone know? Your help is deeply appreciated. Thanks in
