[Ger-Poland-Volhynia] Request for help reading a letter in Polish

Dear Members,

I recently received a letter from Archiwum Panstwowe regarding my request for
information on family members I am trying to research.

I haven't been too successful using the  Polish/English online translators.  I
don't know anyone who can read Polish and was wondering if someone could
translate it for me if I sent a scan?
