[Ger-Poland-Volhynia] chodecz record help - cyrillic

I'm gradually getting better at interpreting Cyrillic in the records, but
need help with this one. It's a Chodecz record from 1904.

Does anyone know what the town name is? - and also the town of his birth?

I think he was 80 something at death (or 18!)
